Registering Death Certificates:

The Mohave County Department of Public Health works with the Office of Vital Records and local funeral homes to register death certificates.  Patty Mead, Director of the Mohave County Health Department, is the Registrar with four Deputy Registrars to assist with services.  Original death certificates are forwarded to the Office of Vital Records within two weeks of receipt.  Information on death certificates that have been forwarded can be given by Arizona Vital Records (602) 255 - 3260 or www.vitalrec.com/az.htm.

Certified Copies:

Certified copies are available to immediate family members at a $10.00 charge per copy - you must present proof of relationship to obtain copies.  If applicable, one certified copy is issued free of charge for social security and one for military benefits.  Business checks, money orders, or exact cash is acceptable for payment of ordered copies.
